(b) In addition to the information specified in Env-Sw 1804.02, the amended certificate shall include the following:

1. (1) The previous composition of the package or packaging component;
2. (2) The reformulation, including the new level of identified heavy metals used; and

3. (3) Any difference(s) between the grounds for compliance as stated in the original certificate of compliance and the amended certificate of compliance.
