1. (a) With the completed, signed application form, the applicant shall submit the fee of $50 authorized by RSA 149-M:6, XIII, as reprinted in Appendix B.
2. (b) The fee, if paid by check or money order, shall be made payable to “Treasurer, State of New Hampshire”.
