“Permittee identification” means all of the following information for the permittee:

1. (a) If an individual, the permittee’s name, permit number, mailing address, daytime telephone number, and email address; or
2. (b) If other than an individual, the name and mailing address of the corporation, partnership, or association, and the authorized contact information.
