“Facility identification” means, in the context of filing an application or registration pursuant to the solid waste rules, all of the following information for a facility:

1. (a) Name;
2. (b) Functional classification pursuant to Env-Sw 302.05;
3. (c) Mailing address;
4. (d) Permit number, if applicable;
5. (e) Location by street address and municipality; and

6. (f) If not yet issued a permit:

   1. (1) Local tax map and lot numbers;
   2. (2) Deed reference by county, volume and page numbers;
   3. (3) A plotting on a United States Geological Survey (USGS) topographic map, or copy thereof, prepared at a scale of 1:24,000 or 1:25,000;
   4. (4) Latitude and longitude of a known fixed point on the site; and
   5. (5) Written directions from a known point of reference in the vicinity of the facility site.
