1. (a) The corrective action plan submitted by the permittee pursuant to Env-Or 703.17(f)(2) shall include the following:

   1. (1) Inspection and audit of activities and procedures at the facility to determine possible sources of contamination;
   2. (2) Remediation of the source of the exceedance;
   3. (3) Further groundwater investigation;
   4. (4) Modification of facility operation as needed to eliminate the cause of the exceedance;
   5. (5) Treatment of the waste stream as needed to eliminate the cause of the exceedance;
   6. (6) Groundwater restoration; and
   7. (7) If the facility operations cannot be modified to eliminate the cause of the exceedance or if the groundwater cannot be restored or remediated, a schedule of activities that will be implemented for facility closure, or a proposal for alternative corrective action plan completion criteria for approval by the department.
2. (b) The corrective action plan shall be dated, signed, and sealed by the professional of record.
