The maps required by Env-Or 703.02(a)(1) shall be legible and include the following:

1. (a) A U.S. Geological Survey (USGS) map, 7-1/2 minute series, marked to clearly identify the facility location; and

2. (b) A potential receptors map using a tax map as a base, marked to identify and locate, to the extent ascertainable, the following:

   1. (1) Streets within 1,000 feet of the facility;
   2. (2) Properties, including tax map and lot numbers, ownership, and land use information, within 1,000 feet of the facility;
   3. (3) Physical structures, storage areas, and buildings, including information on building use and existence of basements, within 1,000 feet of the facility;
   4. (4) Surface waters within 1,000 feet of the facility; and
   5. (5) Water supply wells, including type of use, within 1,000 feet of the facility.
