The site investigation report shall include a preliminary screening of remedial alternatives, a summary, and recommendations including:

1. (a) A preliminary screening of remedial alternatives that identifies remedial action alternatives that meet the criteria of Env-Or 606.13.

2. (b) A summary of the following:

   1. (1) Site conditions;
   2. (2) Subsurface explorations;
   3. (3) Conceptual model including contaminant distribution; and
   4. (4) Receptors and potential receptors; and

3. (c) Recommendations, including a discussion of proposed corrective action activities, that include one or more of the following as appropriate:

   1. (1) Interim response actions to abate immediate risks to human health and the environment;
   2. (2) Periodic sampling;
   3. (3) Additional site investigation work;
   4. (4) Remedial action; or
   5. (5) No further action.
