1. (a) Unless exempted under Env-Or 604.03, the responsible party who becomes aware of the groundwater quality violation shall notify the department no more than 60 days after obtaining knowledge of a violation of the groundwater quality criteria of Env-Or 603.01.

2. (b) Any person required to notify the department pursuant to (a), above, shall provide notification to the department in writing, including as much of the following information as is available at the time of notification:

   1. (1) The name, daytime telephone number, and email address, if any, of the individual notifying the department;
   2. (2) The location of the site;
   3. (3) The nature and location of the violation;
   4. (4) The name, daytime telephone number, and email address, if any, of each responsible party, and if there is more than one responsible party, the name, daytime telephone number, and email address, if any, of a point of contact;
   5. (5) The proximity of the violation to receptors and potential receptors including water supply wells and surface water; and
   6. (6) All available reports and sampling results related to the discovery of the violation.
