1. (a) For failing to keep the manifest copies identified in Env-Hw 512.01(a)(1) for the period specified, the proposed fine shall be $750 per copy certified by the designated facility and $250 per original generator copy.
2. (b) For failing to keep the records identified in Env-Hw 512.01(a)(2) through (12) for the period specified, the proposed fine shall be $250 per record.
3. (c) For failing to notify the department of any needed corrections within 30 days of receipt of a quarterly activity report or failing to certify the report in violation of Env‑Hw 512.02(e)(2) and (3), the proposed fine shall be $250 per quarterly report.
4. (d) For failing to submit a certified copy of the quarterly activity report in violation of Env-Hw 512.02(e)(4), the proposed fine shall be $750 per certified copy of a quarterly report not submitted.
