A transporter shall notify the department by providing updated information in writing within 15 days of any of the following:

1. (a) A name change for the transporter;
2. (b) A change in physical location or mailing address of the transporter;
3. (c) A change of company contact of the transporter;
4. (d) A change in telephone number of the transporter; and
5. (e) A change in the EPA identification number of the transporter.
