1. (a) The applicant shall submit:

   1. (1) The original and 3 copies of the application and all supporting materials required by Env-Hw 304.10;
   2. (2) A cover letter signed by the applicant certifying, as specified in Env-Hw 207, that the submitted application meets all necessary application requirements sufficient to render it technically adequate to undergo an engineering review; and
   3. (3) The fee required by Env-Hw 304.07.
2. (b) An applicant who wishes to apply for a waiver to any provision of Env-Hw 304.09(e) shall do so pursuant to Env-Hw 202 at the time the application is submitted. The department shall address the applicant’s waiver request within its site evaluation.
