1. (a) A large quantity handler or very large quantity handler shall keep a record, which may take the form of a log, invoice, manifest, bill of lading, movement document, or other shipping document, of each shipment of universal waste sent by the universal waste handler to another facility.

2. (b) The record for each shipment of universal waste sent shall include the following information:

   1. (1) The name and address of the universal waste handler, destination facility, or foreign destination to which the universal waste was sent;
   2. (2) The quantity of each type of universal waste; and
   3. (3) The date the shipment of universal waste left the universal waste handler's facility.
3. (c) The records required by (a) and (b), above, shall be retained for at least 3 years from the date the universal waste shipment to which it relates left the universal waste handler's facility.
