1. (a) A large quantity handler or a very large quantity handler shall notify the department pursuant to (b), below, before accumulating the following quantities of universal waste:

   1. (1) For large quantity handlers, 5,000 kilograms or more but less than 20,000 kilograms; or
   2. (2) For very large quantity handlers, 20,000 kilograms or more.

2. (b) A large quantity handler or a very large quantity handler shall complete and submit:

   1. (1) Form HWM-2 “RCRA C Site Identification Form” as amended August 2025; and
   2. (2) The following certification: “I certify, in accordance with Env-Hw 207, under penalty of law that this document and all attachments were prepared under my direction or supervision in accordance with a system designed to assure that qualified personnel properly gather and evaluate the information submitted. Based on my inquiry of the person or persons who manage the system, or those persons directly responsible for gathering the information, the information submitted is, to the best of my knowledge and belief, true, accurate, and complete. I am aware that there are significant penalties for submitting false information, including the possibility of fines and imprisonment for knowing violations.”
3. (c) A large quantity handler or very large quantity handler shall notify the department in accordance with (a), above, for each onsite location where universal waste is accumulated.
4. (d) Upon receipt of a complete, signed notification, the department shall issue an EPA identification number to the universal waste handler if the universal waste handler does not already possess an EPA identification number.

5. (e) The EPA identification number assigned pursuant to (d), above, shall:

   1. (1) Be site specific; and
   2. (2) Remain valid until the universal waste handler notifies the department in writing that universal waste is no longer being accumulated at the site.

6. (f) A large quantity handler or very large quantity handler shall notify the department of any changes to the information submitted pursuant to (a), above, within 30 days of the effective date of the change, as follows:

   1. (1) Notify orally or in writing of any changes to:

      1. a. Site’s legal name;
      2. b. Mailing address;
      3. c. Name, title, telephone number, and, if available, email address of a contact person; or
      4. d. Type and description of regulated activity; and
   2. (2) Notify on Form HWM-2 “RCRA C Site Identification Form” as amended August 2025, of any changes to information other than those specified in (f)(1), above.
7. (g) Unless the universal waste handler becomes a very large quantity handler, the designation as a large quantity handler shall be retained through the end of the calendar year in which 5,000 kilograms or more total of universal waste is accumulated at any one time.
8. (h) The designation as a very large quantity handler shall be retained through the end of the calendar year in which greater than or equal to 20,000 kilograms total of universal waste is accumulated at any one time.
