Any of the following conditions shall constitute a significant deficiency:

1. (a) Well casing is buried;
2. (b) Top of well can be flooded;
3. (c) Well is not sealed;
4. (d) Unsanitary material or debris is stored or discarded in the sanitary radius of a well;
5. (e) Well is not constructed in accordance with applicable requirements;
6. (f) Well casing extension does not meet applicable requirements;
7. (g) Well is damaged or otherwise subject to contamination;
8. (h) Operation, maintenance, or monitoring of disinfection processes does not meet applicable requirements;
9. (i) System has low or negative pressures that could result in entry of contaminants;
10. (j) System lacks cross connection control program or devices, if applicable;
11. (k) Pump house is subject to flooding;
12. (l) Pump house conditions are unsanitary;
13. (m) System does not have a certified operator even though one is required by Env-Dw 502;
14. (n) Certified operator, if required, is below the appropriate grade;
15. (o) Inspection, internal cleaning, and maintenance of storage tank(s) does not meet applicable requirements;
16. (p) Water storage tank is subject to contamination;
17. (q) Facility operations are not inspected for unsanitary conditions on a regular basis;
18. (r) Monitoring, reporting, and recordkeeping are not being done as required; and
19. (s) Any other significant deficiency identified in Env-Dw 720.03.
