1. (a) All suppliers of water identified in Env-Dw 505.02(a) shall submit an annual inspection and testing report to the department in accordance with this section.
2. (b) The annual inspection and testing report shall be submitted to the department no later than April 1st of the year following the inspection year.

3. (c) The annual inspection and testing report shall:

   1. (1) Specify the total number of permitted cross connections at the end of the year for which the report is being filed;
   2. (2) Specify the number of backflow prevention devices tested; and

   3. (3) Provide the following information for each backflow prevention device:

      1. a. Permit number of the backflow prevention device;
      2. b. Name of the owner of the backflow prevention device;
      3. c. Location of the backflow prevention device;
      4. d. Date of each inspection and test performed during the year of reporting;
      5. e. Name, certifying organization, and certification number of the certified backflow prevention device inspector who performed the inspection and test on the device; and
      6. f. If the inspection or test result is unsatisfactory, the date of correction.
4. (d) A supplier of water with over 1,000 service connections that demonstrates the absence of any connection(s) with a high degree of hazard shall be exempt from reporting.
