1. (a) The owner of a new PWS or a new owner of an existing PWS shall provide the following information to the department:

   1. (1) Name and location, including the street address and map and lot number(s) of the propert(ies) that contain(s) the water supply source(s), of the system; and

   2. (2) The information required in (b) below for each of the following individuals or entities:

      1. a. The owner's name;
      2. b. The owner’s contact person, if different from the person specified in (a)(2)e. below;
      3. c. If applicable, the owner’s representative;
      4. d. If applicable, the primary certified operator designated in accordance with Env-Dw 503.02(b);
      5. e. The sampling agent, if different from the person specified in (a)(2)f. below; and
      6. f. If applicable, the emergency designee as required in Env-Dw 503.04.
2. (b) The notice provided pursuant to (a)(2) above, shall include the name of each individual or entity, mailing address, daytime telephone number, emergency telephone number, and the email address that the person uses for the PWS’s business.
