1. (a) The department shall record the information described in (b) or (c), below, as applicable, into its database:

   1. (1) For all existing CWS or NTNC; and
   2. (2) When a PWS ID is assigned by the department to any new CWS or NTNC.

2. (b) For an NTNC, the department shall record:

   1. (1) The name and location of the NTNC;
   2. (2) The name, mailing address, and daytime telephone number of the owner of the NTNC and, if available, an e-mail address; and
   3. (3) An estimate of the number of persons served by the system on a repeat basis for at least 6 months per year.

3. (c) For a CWS, the department shall record:

   1. (1) The name and location of the CWS;
   2. (2) The name, mailing address, and daytime telephone number of the owner of the CWS and, if available, an e-mail address;
   3. (3) An estimate of the number of persons served by the water system; and
   4. (4) The number of household service connections in the CWS.
