(a) Any person seeking to expand a non-community PWS shall submit the following to the department:
- (1) Historical water use data for at least the past 2 years;
- (2) The proposed design flow calculated in accordance with Env-Dw 406.11; and
- (3) Documentation demonstrating the existing source(s) have the required supply capacity as required by Env-Dw 406.12.
(b) Any person seeking to modify or expand a PWS in accordance with Env-Dw 406.05 shall submit the following to the department:
- (1) A written request to amend the design approval;
- (2) A description of the necessary changes;
- (3) An explanation of why the changes are needed;
(4) Plan sheets, including revisions to previously approved plans, and the following, if applicable:
- a. For new well(s), the information in Env-Dw 406.13 through 406.16;
- b. For storage tank(s), the information in Env-Dw 406.20;
- c. For pump controls and auxiliary equipment, the information in Env-Dw 406.18;
- d. For a pumphouse or pump room, the information in Env-Dw 406.17;
- e. For well appurtenances and pumps, the information in Env-Dw 406.19;
- f. For wiring and control devices, the information in Env-Dw 406.21;
- g. For water treatment facilities, the information in Env-Dw 406.22, 406.23, and 406.25; and
- h. For water distribution systems, the information in Env-Dw 406.26;
- (5) Revised calculations, as applicable;
- (6) If infrastructure is decommissioned, photo documentation showing a minimum 12-inch gap in the piping from the original well(s), storage tank(s), and any other decommissioned infrastructure;
(7) If connection to another PWS is proposed, documentation from the supplying owner certifying that:
- a. Adequate quantities of water are available to serve the proposed PWS; and
- b. With the proposed PWS online, adequate system pressures and source capacity will be maintained; and
- (8) The design fee as required by Env Dw-402.03.
(c) For deepening or hydrofracturing of wells, the applicant shall submit:
- (1) The well capacity after hydrofracturing or deepening determined pursuant to Env-Dw 406.15; and
- (2) The water quality analysis results of the water from each well as specified in Env-Dw 406.16.
- (d) Replacement in-kind does not require department approval, except that replacement of a well shall comply with the requirements of Env-Dw 406.13 through Env-Dw 406.16.
- (e) The plans and specifications for modifications to PWSs having a design flow greater than 20,000 gpd shall be sealed by a professional civil or sanitary engineer licensed in the state of New Hampshire.
- (f) The department shall approve the application to modify the non-community PWS if the design meets the criteria in this part.
