(a) An applicant requiring final design of a non-community water system, including reactivation or registration, shall provide the following information constituting part II of the application:
- (1) A corrected copy of part I of the application previously submitted, which clearly identifies any changes from the original submittal, if applicable;
- (2) A revised plan, if different than that submitted with part I of the application pursuant to Env-Dw 406.06(a);
- (3) The well completion report for each well as required in We 800;
- (4) The water quality analysis results of the water from each well as specified in Env-Dw 406.16;
- (5) The well capacity of each well proposed to be connected to the PWS determined pursuant to Env-Dw 406.15;
- (6) The name of a designated primary operator certified in accordance with Env-Dw 502 or a person that has taken or arranged to take the operator certification exam as required under Env-Dw 502;
- (7) For NTNC water systems, a business plan pursuant to Env-Dw 602;
- (8) An emergency plan pursuant to Env-Dw 503;
(9) The following design information, as applicable:
- a. For storage tank(s), the information in Env-Dw 406.20;
- b. For pump controls and auxiliary equipment, the information in Env-Dw 406.18;
- c. For a pumphouse or pump room, the information in Env-Dw 406.17;
- d. For well appurtenances and pumps, the information in Env-Dw 406.19;
- e. For wiring and control devices, the information in Env-Dw 406.21;
- f. For water treatment facilities, the information in Env-Dw 406.22, Env-Dw 406.23, and Env-Dw 406.25; and
- g. For water distribution systems, the information in Env-Dw 406.26; and
- (10) For well source(s) constituting a large groundwater withdrawal as defined in RSA 485-C:2, a copy of the large groundwater withdrawal permit issued by the department in accordance with Env-Wq 403.
- (b) Any material, product, or equipment incorporated into a PWS shall comply with the appropriate standard(s) of the AWWA, as specified in Env-Dw 402 if such a standard exists, and shall be specified in the design information accordingly.
- (c) The applicant shall sign and date the application to certify that the information provided is accurate.
- (d) The design review fee required by Env-Dw 402.03 shall be paid in conjunction with submission of part II of the application.
