- (a) An applicant requesting approval to construct or register a new PWS or reactivate a PWS shall submit a complete application, as specified in (b) below, to the department electronically at dwengineering@des.nh.gov.
(b) An application shall be submitted in two parts, part I, concept approval, and part II, final design:
- (1) Part I shall be completed as specified in Env-Dw 406.06 and part II shall be completed as specified in Env-Dw 406.09; and
- (2) Part II shall not be submitted until part I has been approved by the department and all new sources required as part of the design have been approved by the department.
(c) An applicant requesting approval to modify, expand, or enlarge an existing PWS shall submit the following to the department electronically at dwengineering@des.nh.gov:
- (1) The name, location, and PWS identifier;
- (2) The name, mailing address, daytime telephone number, and e-mail address of the contact person who can answer questions about the proposal on behalf of the owner; and
- (3) The information required in Env-Dw 406.29.
- (d) The plans and specifications for all proposed non-community water systems or modifications, including expansions, to existing non-community water systems with a design flow greater than 20,000 gallons per day (gpd) or with 50 or more service connections shall be stamped by a professional engineer licensed in the state of New Hampshire.
