(a) Any person seeking to expand or modify a small CWS shall submit the following to the department:
- (1) Historical water use data for at least the past 2 years;
- (2) The proposed design flow calculated in accordance with Env-Dw 405.11;
- (3) Documentation demonstrating the existing source(s) have the required supply capacity as required by Env-Dw 405.13;
- (4) A written request to amend the design approval;
- (5) A description of the necessary changes;
- (6) An explanation of why the changes are needed;
(7) For any new wells:
- a. The well driller’s well completion report for each well as required in We 800;
- b. The water quality analysis results of the water from each well as specified in Env-Dw 405.15; and
- c. The permitted production volume of each well proposed to be connected to the small CWS, approved pursuant to Env-Dw 302, Env-Dw 305, or both as applicable;
(8) Plan sheets, including revisions to previously approved plans, and the following, if applicable:
- a. For storage tank(s), the information in Env-Dw 405.16 and Env-Dw 405.21;
- b. For booster pumps, pump controls, and auxiliary equipment, the information in Env-Dw 405.17 and Env-Dw 405.19;
- c. For a pumphouse or pump room, the information in Env-Dw 405.18;
- d. For well appurtenances and pumps, the information in Env-Dw 405.20;
- e. For wiring and controls, the information in Env-Dw 405.22;
- f. For water treatment facilities, the information in Env-Dw 405.23, Env-Dw 405.24, and Env-Dw 405.26; and
- g. For water distribution systems, the information in Env-Dw 405.27;
- (9) Revised calculations, as applicable;
- (10) If infrastructure is decommissioned, photo documentation showing a minimum 12-inch gap in the piping from the original well(s), storage tank(s), and any other decommissioned infrastructure; and
(11) If connection to another PWS is proposed, documentation from the supplying owner certifying that:
- a. Adequate quantities of water are available to serve the proposed PWS;
- b. With the proposed PWS online, adequate system pressures and source capacity will be maintained; and
- c. The design fee as required by Env-Dw 402.03.
- (b) For deepening or hydrofracturing of wells at CWS, the applicant shall comply with Env-Dw 302 and Env-Dw 305, as applicable.
- (c) Replacement in-kind does not require department approval, except that replacement of a well shall comply with the requirements of Env-Dw 302 or Env-Dw 305, as applicable.
- (d) The plans and specifications for modifications to PWSs having a design flow greater than 20,000 gpd shall be sealed by a professional civil or sanitary engineer licensed in the state of New Hampshire.
- (e) The department shall approve the application to modify the CWS if the design meets the criteria in this part.
