1. (a) An applicant requesting approval to construct or register a new small CWS or reactivate a small CWS shall submit a complete application, as specified in (b) below, to the department electronically at dwengineering@des.nh.gov.

2. (b) An application shall be submitted in 2 parts, part I, concept approval, and part II, as follows:

   1. (1) Part I shall be completed as specified in Env-Dw 405.06, and part II shall be completed as specified in Env-Dw 405.09;
   2. (2) Part I of the design review application required by Env-Dw 405.06 shall be submitted and approved by the department prior to submission of the preliminary well report specified in Env-Dw 302 or Env-Dw 305; and
   3. (3) Part II shall not be submitted until part I has been approved by the department and all new sources required as part of the design have been approved by the department in accordance with Env-Dw 302, Env-Dw 305, or both, as applicable.

3. (c) An applicant requesting approval to modify, expand, or enlarge an existing small CWS shall submit the following to the department electronically at dwengineering@des.nh.gov:

   1. (1) The name, location, and PWS identifier;
   2. (2) The name, mailing address, daytime telephone number, and e-mail address of the contact person who can answer questions about the proposal on behalf of the owner; and
   3. (3) The information required in Env-Dw 405.31.
4. (d) The plans and specifications for all proposed small CWSs or modifications, including expansions, to existing small CWSs with a design flow greater than 20,000 gpd or with 50 or more service connections shall be sealed by a professional engineer licensed in the state of New Hampshire.
