1. (a) Any final design approval for large CWS designs shall expire 4 years after issuance if construction has not been completed.
2. (b) A new design review fee and new design submission including design revision, as applicable, shall be required for review and re-approval of lapsed designs. The design shall meet the requirements in effect at the time of the resubmittal.

3. (c) To request an extension to a large CWS design approval, the owner shall submit a written request for extension, including:

   1. (1) Specific reason(s) why the extension is being sought;
   2. (2) The new construction schedule;
   3. (3) Confirmation that no changes have been made to the original approved design; and
   4. (4) A new water quality test for each source for the applicable parameters listed in Env-Dw 701 through Env-Dw 706.
4. (d) After reviewing an extension request, if the department finds that the previously approved design will ensure that the project is consistent with all applicable requirements in place at the time the extension request was received, the department shall extend the approval for a reasonable amount of time, under the circumstances, not to exceed 4 years.
