1. (a) Any person seeking to expand a large CWS shall submit the following to the department:

   1. (1) A water audit conducted in accordance with the standards of the AWWA;
   2. (2) The proposed increase in average day and maximum day demand due to the proposed expansion; and
   3. (3) Documentation demonstrating the existing source(s) have the required supply capacity as required by Env-Dw 404.04 and Env-Dw 404.05.

2. (b) Any person seeking to modify or expand a large CWS in accordance with Env-Dw 404.02 above shall submit the following to the department:

   1. (1) A description of the proposed modifications or expansions;
   2. (2) An explanation of why the modifications or expansions are needed;
   3. (3) Plan sheets, including revisions to previously approved plans, and specifications for the proposed modifications or expansion as required by Env-Dw 404.04 and Env-Dw 404.05;
   4. (4) Revised calculations, as applicable;
   5. (5) If infrastructure is decommissioned, photo documentation showing a minimum 12-inch gap in the piping from the original well(s), storage tank(s), and any other decommissioned infrastructure;

   6. (6) If connection to another PWS is proposed, documentation from the supplying owner certifying that:

      1. a. Adequate quantities of water are available to serve the proposed PWS; and
      2. b. With the proposed PWS online, adequate system pressures and source capacity will be maintained in the supplying PWS; and
   7. (7) The application fee as required by Env-Dw 402.03.
3. (c) For deepening or hydrofracturing of wells at CWSs, the applicant shall comply with Env-Dw 302 and Env-Dw 305, as applicable.
4. (d) Replacement in-kind shall not require department approval, except that replacement of a well shall comply with the requirements of Env-Dw 302 or Env-Dw 305, as applicable.
5. (e) The plans and specifications for modifications to large CWSs shall be sealed by a professional engineer licensed in the state of New Hampshire.
6. (f) The department shall approve the application to modify the CWS if the design meets the criteria in this part.
