(a) An applicant requesting approval to construct or register a large CWS pursuant to this part shall submit the following information to the department electronically at dwengineering@des.nh.gov:
- (1) An engineer’s report, design drawings, and specifications as specified in Env-Dw 404.04 and Env-Dw 405.05;
- (2) The well driller’s well completion report for each well as required in We 800;
- (3) The water quality analysis results of the water from each source as specified in Env-Dw 302;
- (4) The permitted production volume for each well, approved pursuant to Env-Dw 302;
(5) If connection to another PWS is proposed, the supplying owner shall provide information demonstrating:
- a. Adequate quantities of water are available to serve the receiving PWS; and
- b. With the receiving PWS online, adequate system pressures and source capacity will be maintained within the supplying PWS;
- (6) The name of a designated primary operator certified in accordance with Env-Dw 502 or a description of a plan and schedule to ensure the presence of a certified primary operator upon commencement of operation of the PWS;
- (7) Business plan pursuant to Env-Dw 602.03;
- (8) Water ordinance or water system rules pursuant to Env-Dw 503.08; and
- (9) An emergency plan pursuant to Env-Dw 503.21.
(b) The applicant shall:
- (1) Ensure that all submitted engineering reports, plans, specifications, calculations, and other pertinent information relating to the design or construction of a large CWS are sealed by a professional engineer licensed in the state of New Hampshire;
- (2) Sign and date the application; and
- (3) Certify that the information provided is accurate.
- (c) The design review fee required by Env-Dw 402.03 shall be paid in conjunction with submission of the application.
(d) An applicant requesting approval to modify, expand, or enlarge an existing large CWS shall submit the following to the department electronically at dwengineering@des.nh.gov:
- (1) The name, location, and PWS identifier;
- (2) The name, mailing address, daytime telephone number, and e-mail address of the contact person who can answer questions about the proposal on behalf of the owner; and
- (3) The information required in Env-Dw 404.08.
- (e) The plans and specifications for all proposed large CWSs or modifications, including expansions, to existing large CWSs shall be sealed by a professional engineer licensed in the state of New Hampshire.
