1. (a) Any person seeking approval pursuant to Env-Dw 302.04(a) shall first submit a preliminary report to the department that contains the following:

   1. (1) The cover page specified in Env-Dw 302.06, which has been signed as specified in Env-Dw 302.07; and
   2. (2) The information and maps identified in Env-Dw 302.08.

2. (b) The preliminary report submitted pursuant to (a), above, shall be:

   1. (1) Dated, signed, and sealed by the professional of record;
   2. (2) Submitted with a preliminary application prepared in accordance with Env-Wq 403 if the new well meets the definition of a large groundwater withdrawal; and
   3. (3) Submitted to the department electronically in a portable document format.
3. (c) The department shall review the preliminary report as specified in Env-Dw 302.16.
4. (d) If the department approves the preliminary report and the applicant wishes to continue, the applicant shall proceed as specified in Env-Dw 302.18.
