An applicant who is or would be directly and adversely affected by the strict application of a rule in this Chapter may submit a waiver request by:

1. (a) Providing all information required by Env-Dw 1507.03, either in writing or on form NHDES-S-03-013;
2. (b) Signing and certifying the waiver request as required by Env-Dw 1507.04; and

3. (c) Submitting the completed waiver request in writing to pfasrebateprogram@des.nh.gov or:
