The person requesting the waiver shall provide the following information to the department:

1. (a) The name, mailing address, and daytime telephone number including area code of the requestor;
2. (b) The eligibility request, application, or loan to which the waiver request relates; and

3. (c) For each waiver sought, the following:

   1. (1) The number of the specific section of the rules;
   2. (2) A complete explanation of why a waiver is being requested, including an explanation of the economic and operational consequences of complying with the rule as written;
   3. (3) If applicable, a full explanation of the alternate(s) proposed to be substituted for the requirement in the rule, including documentation or data, or both, to support the alternative;
   4. (4) Whether the need for the waiver is time-limited and, if so, the estimated length of time the waiver will be needed; and
   5. (5) A complete explanation of why the applicant believes that having the waiver granted will meet the criteria in Env-Dw 1408.05.
