The ownership information required by Env-Dw 303.03(d) shall be as follows:

1. (a) Whether the laboratory seeking accreditation is owned by a business entity or a government entity;

2. (b) If the laboratory is owned by a business entity, the following:

   1. (1) The type of business entity, such as corporation, a limited liability corporation, a partnership, a sole proprietorship, or another type, which shall be identified;
   2. (2) The legal name of the business entity, if other than the legal name of the laboratory;
   3. (3) The date the laboratory registered with the New Hampshire secretary of state;
   4. (4) If the owner is a corporation, the date and state of incorporation; and
   5. (5) The name and title of each principal official of the business entity, such as corporate officers or general partners; and
3. (c) If the laboratory is owned by a governmental entity, the name and primary mailing address of the federal, state, or local agency that owns the laboratory, if different from the information provided pursuant to Env-C 303.04(a).
