The person filing a motion for reconsideration shall provide the following information:

1. (a) The exact legal name of each person requesting reconsideration and the mailing address of the person and, if available, a fax number and e-mail address for the person;
2. (b) If the person making the request is other than an individual, the name and daytime telephone number of the individual who can be contacted regarding the motion and, if available, a fax number and e-mail address for that individual;
3. (c) A clear and concise statement of the reason(s) why the person believes the decision to be in error;
4. (d) A clear and concise statement of the facts upon which the department is expected to rely in granting relief;
5. (e) A clear and concise statement of the specific relief or ruling requested;
6. (f) A copy of the decision that is the subject of the motion; and
7. (g) Such other information as the party filing the motion deems pertinent and relevant, including sworn written testimony and other evidence that was not available for the hearing.
