1. (a) A certification holder whose certification has not yet expired or has been expired for less than 3 years, shall submit to the department, on or with the form, as required by Env-A 1818.05, obtained from the department, the following information:

   1. (1) All application information required in Env-A 1818.04(c);
   2. (2) The certification number and issue date of the applicant’s expiring or expired certification;

   3. (3) A list of asbestos disposal site projects that the certification holder has worked on since the date the expiring or expired certification was issued, including:

      1. a. The site location;
      2. b. The name of the property owner;
      3. c. The name, address, and telephone number of the license holder for whom the certification holder performed the work;
      4. d. The name, address, and telephone number of the project site supervisor;
      5. e. The date the project commenced and the date the project was completed; and
      6. f. The nature of the work performed;
   4. (4) A statement by the applicant attesting that the annual refresher training requirements specified in Env-A 1819.05 have been satisfied;

   5. (5) A non-refundable application fee for each application as follows:

      1. a. For applications submitted in calendar year 2026, $75; and
      2. b. For applications submitted in calendar year 2027 and each year thereafter, $100; and
   6. (6) A statement by the applicant attesting that all of the requirements specified in Env-A 1818.09(c) have been met.
2. (b) The applicant shall sign and date the application as specified in Env-A 1818.04(d).
3. (c) The applicant shall submit an application, as specified in (a) and (b) above, to the department no more than 90 days prior to the expiration date of the certification.
4. (d) The department shall review and process applications in accordance with Env-A 1818.05.
5. (e) The department shall not approve an application with an expiring certification that is under suspension until the applicant fully satisfies all terms and conditions of the suspension, in accordance with Env-A 1818.06(f).
