If a permit is required to be obtained or modified in accordance with Env-A 1405.04, the owner or operator of such source shall:

1. (a) File an application for such permit or permit modification, in accordance with Env-A 612, within 90 days following publication of notice of such final revision in the New Hampshire Rulemaking Register; and

2. (b) Submit a compliance plan based on public health, and economic and technical considerations that shall contain, at a minimum, the following information:

   1. (1) The name and CAS registry number for each RTAP emitted from each process or device subject to the compliance plan;
   2. (2) The date upon which compliance with the applicable provisions will be achieved, which shall be no later than 3 years following publication of notice of such final revisions in the New Hampshire Rulemaking Register;
   3. (3) A list of remedial measures, including the sequence of actions or operations with milestones for each action; and
   4. (4) A schedule for the submission of progress reports to the department.
