1. (a) Each sewer utility shall complete and file with the department and the commission a completed department “Form F-22, Information Sheet” revised October 2023, available on the department’s website at www.energy.nh.gov:

   1. (1) Annually; and
   2. (2) Whenever any changes occur to the information included in the form F-22 filing.
