1. (a) Each utility shall file department "Form F-22, Information Sheet," dated October 2023 with the department and the commission:

   1. (1) Annually; and
   2. (2) Whenever any changes occur to the information included in the Form F-22 filing.

2. (b) Each utility shall include the following on Form F-22:

   1. (1) The name of the utility filing the report;
   2. (2) Person's name, title, and e-mail address to receive the annual report form;
   3. (3) Person's name, title, and e-mail address to receive the utility assessment tax;
   4. (4) The names and titles of the principal officers of the company; and
   5. (5) The signature, full name and title of the utility employee who supervised the preparation of the report, and date of signature.
