“Significant facility disruption” means an outage of facilities owned, operated, leased, or otherwise utilized by a telephone utility that is at least 30 minutes in duration and that:

1. (a) Potentially affects at least 900,000 user minutes;
2. (b) Affects at least 1,350 DS3 minutes;
3. (c) Potentially affects any special offices and facilities listed in 47 C.F.R. § 4.5(b); or
4. (d) Potentially affects a 911 special facility as defined in 47 C.F.R.§ 4.5(e).
