Each approved program shall annually, in October, submit to the state board a report including, at a minimum, the following:

1. (a) A brief statement explaining how the program is meeting the goals of its mission statement; and
2. (b) The number of students enrolled in the program and the number of students awarded certificates leading to high school credits for the previous school year.
