1. (a) The NHVR counselor shall:

   1. (1) Collect, analyze, synthesize, and interpret available information;
   2. (2) Engage the individual with a disability in the assessment process; and
   3. (3) Develop a positive counseling and guidance relationship by making every effort to understand the individual’s priorities during the assessment process.
2. (b) If additional information is needed, the NHVR counselor, along with the individual receiving services, shall decide how to obtain the information and establish appropriate timeframes for completion.
3. (c) The decision to pursue any additional assessments shall be fully discussed and explained to the individual.
4. (d) The NHVR counselor shall obtain only the information necessary for eligibility or service planning.
5. (e) The individual’s active involvement shall be documented in the record of services.
