The OPLC shall reinstate licenses lapsed for no more than one year if the affected individuals:
(a) Are of good professional character, as evidenced by:
- (1) The answers to the “yes-no” questions on the reinstatement application form; and
- (2) Any detailed reports submitted, indicating relevant circumstances which are related to answers to those questions as described in Plc 308.06(e); and
- (3) The resume described in Diet 403.05(b);
- (b) Have, within the 2 years preceding the submission of the reinstatement application, completed a minimum of 20 credit hours of continuing professional education as described in Diet 404; and
- (c) Have submitted all of the materials as described in Diet 403.06.
