License renewal shall be denied if the OPLC finds that the applicant has:
- (a) Failed to file a renewal application form and all required supporting documentation prior to the date of expiration of the license;
- (b) Failed to submit the fees with the renewal application as required by Plc 1001.08 and Plc 1002.12;
- (c) Failed to complete a minimum of 20 credit hours of continuing professional education in the 2 years prior to the expiration of any license or renewed license;
- (d) Answered “yes” to the question on the renewal application form described at Plc 308.06(e); or
- (e) Failed to fulfill any other condition of licensure, including the terms of any settlement or agreement with the board.
