Procedure for Renewal of Licenses

- (a) Any licensee wishing to renew a license shall do so in accordance with Plc 308.09 through Plc 308.12.
(b) The licensee wishing to renew their license shall submit to the office of professional licensure and certification (OPLC):
- (1) A completed “Universal Application for License Renewal” required in Diet 402.02(a);
- (2) A completed “Dietitian Addendum to the Universal Application for License Renewal” required in Diet 402.02(b); and
- (3) The renewal fees required by Plc 1001.08 and Plc 1002.12.
- (c) A person who was previously licensed by the OPLC but whose license expired pursuant to a settlement or agreement shall apply to have that license reinstated in accordance with the terms of the settlement or agreement and meet any additional requirements required by Diet 403.
- (d) A person who was previously licensed by the OPLC but whose license expired while under suspension shall apply to have that license reinstated in accordance with Diet 403.03 and meet any additional requirements required by Diet 403.
- (e) A person who was previously licensed by the OPLC but whose license has been revoked shall apply to have that license reinstated in accordance with the terms of the revocation in accordance with Diet 403.04 and meet any additional requirements required by Diet 403.
- (f) A licensee may apply for renewal of a license during a period of suspension, but any renewed license shall be issued subject to completion of the term of suspension.
