1. (a) Each person who wishes to apply for a license to practice as a dietitian in New Hampshire shall do so by submitting the following:

   1. (1) The “Universal Application for Initial Licensure” as required by Plc 304.01(a)(1);
   2. (2) The “Dietitian Addendum to the Universal Application for Initial Licensure” as required in Diet 302.02;
   3. (3) All additional supporting materials specified by Diet 302.03; and
   4. (4) The license fee required by Plc 1002.12 and the fee required by Plc 1001.08.
2. (b) An application form shall be processed in accordance with Plc 304.06 through Plc 304.11.
