1. (a) Any person may request a declaratory ruling from the commissioner on matters within the person’s jurisdiction by filing an original and 5 copies of a petition.

2. (b) All petitions shall contain:

   1. (1) The name and address of the petitioner;
   2. (2) The name and address of the petitioner’s representative, if any;
   3. (3) A concise statement of the facts that caused the petitioner to request the commissioner to act;
   4. (4) The action that the petitioner wishes the commissioner to take; and
   5. (5) The identification of any statutes, rules, orders, or other authority that entitles the petitioner to request the commissioner to act.

3. (c) A petition for declaratory ruling shall set forth the following information:

   1. (1) The exact ruling being requested; and
   2. (2) The statutory and factual basis for ruling, including any supporting affidavits or memoranda of a law.
