1. (a) A person may request the board to commence a proceeding for the purpose of adopting, amending or repealing a rule by filing a written petition containing:

   1. (1) A statement of the petitioner’s request for the proposed rules;
   2. (2) The text of the proposed rule or a statement of the results intended by the petitioner’s interest in the subject matter of the proposed rule;
   3. (3) The identification of the specific rule sought to be amended or repealed, if applicable;
   4. (4) A statement whether, in regard to the petitioner, the issue which is the subject of the petition is pending in litigation;
   5. (5) Any data or argument the petitioner believes would be useful to the board in deciding whether to commence a rulemaking proceeding; and

   6. (6) Petitioner’s:

      1. a. Dated signature; and
      2. b. Mailing address.

2. (b) The petition shall be filed with:

   “Rules”
