The trustee shall accept contributions to an account by a person only if such person has completed and executed a participation agreement and account application form requesting such information as necessary and appropriate for the purposes of the plan. Contributions to an account made by a third party shall be accepted but shall be treated for purposes of the plan as though they had been made by the person who has executed a participation agreement with respect to the account.
