The claimant shall file the claim by submitting:

1. (a) To the agency complained against, one copy of the completed claim form and supporting documents; and

2. (b) To the secretary of state at the address stated in (c) below:

   1. (1) Three copies of the completed claim form and supporting documents; and
   2. (2) If the claim is for an amount greater than $5,000.00, payment of a fee in the amount of $25.00.
