At least 10 days before a competition for which a permit has been granted, the promoter shall submit the following additional information:

1. (1) Evidence of obtaining a performance bond and medical insurance for the competition;
2. (2) Evidence of obtaining authorization from the city or town where the competition will be located;
3. (3) Description of final public safety plan, including arrangements for law enforcement, private security guards, fire department, and emergency medical technicians;
4. (4) Names of 2 medical doctors or physician's assistants who will attend the competition; and
5. (5) Final list of referees, judges, scorekeepers, and timekeepers who will officiate at the competition.
