Renewal Forms

- (a) The OPLC shall send a renewal form at least 60 days prior to the expiration date of registration.
- (b) Each applicant wishing to renew their registration shall complete and file with the OPLC a renewal form at least 30 days prior to the expiration date of a registration and attach the fee as required by Plc 1002.07.
(c) Each applicant shall include the following on the form:
- (1) The name and address of the facility, in accordance with RSA 313-A:28, II;
- (2) The name and address of the owner(s), in accordance with RSA 313-A:28, II;
- (3) A list of equipment including the amount; and
- (4) Signature of owner(s) and current date.
