Licensees shall report the following to the OPLC within 30 days, whether applicable to themselves or other licensees:

1. (a) Any misdemeanor or felony conviction;
2. (b) Disciplinary actions by regulatory authorities other than the New Hampshire governing board of athletic trainers; and
3. (c) Any act which constitutes misconduct under Ath 404.02.
