1. (a) A consumer or lessee shall request a hearing before the board on the demand for arbitration, form CPMVA-2, provided by the manufacturer as specified in RSA 357-D:4, I.
2. (b) A legible photocopy of the request for a hearing, together with legible photocopies of all supporting data filed with the board, shall be sent by the consumer or lessee to the manufacturer or lessor.
3. (c) All parties shall furnish the board with 6 legible photocopies, and all opposing parties with one legible photocopy, of any document they request to be entered as part of the record, no later than 5 working days prior to the date of the scheduled hearing.
