1. (a) Any person may request the council to commence a proceeding for the purpose of adopting, amending, or repealing a rule by filing a written petition that contains:

   1. (1) A statement of the petitioner’s request for the proposed rule;
   2. (2) The text of the proposed rule or a statement of the particular results intended by the petitioner’s interest in the subject matter of the proposed rule;
   3. (3) An identification of the particular rule sought to be amended or repealed;
   4. (4) Any data or argument the petitioner believes would be useful to the council in deciding whether to commence a rulemaking proceeding; and
   5. (5) Name, address, signature of petitioner and date.
